We are seeking an experienced and motivated Hospital Liaison to join our long-term care team. The Hospital Liaison serves as the key connection between hospitals, referral sources, residents, and our facility. This role is responsible for building strong relationships with hospital discharge planners, physicians, case managers, and community partners to ensure seamless transitions of care for patients entering our skilled nursing and rehabilitation programs. Act as the primary contact for hospitals, rehabilitation centers, and other referral sources regarding patient admissions. Assess potential referrals to ensure appropriate placement, collaborating with clinical staff for review of medical and care needs. Educate hospital staff, patients, and families about facility services, programs, and levels of care. Coordinate pre-admission screenings, tours, and communication between referral sources and facility leadership. Track referral activity, admissions, and denials; report trends to leadership to guide census development. Represent the facility in hospitals and the community, attending events, health fairs, and meetings as our Hospital Liaison. Develop and maintain strong professional relationships with physicians, discharge planners, and community partners. Support census and occupancy goals by proactively marketing facility programs and specialized services. Ensure compliance with all state, federal, and company policies regarding admissions and resident rights. Compensation and Schedule

Bachelors degree in healthcare, marketing, social work, or related field preferred; equivalent experience considered. Minimum of 2 years experience in healthcare admissions, Hospital Liaison, case management, or long-term care. Strong knowledge of Medicare, Medicaid, and insurance requirements for skilled nursing and rehabilitation admissions. Excellent interpersonal and communication skills with the ability to build lasting professional relationships. Self-motivated, goal-oriented, and comfortable working independently in hospital and community settings. Reliable transportation and willingness to travel locally between hospitals and the facility.
