Position Overview

This is a general supervisory position within the habilitation care field under the general supervision of the Habilitation Care Supervisor II, or other designated supervisor. This role is responsible for ensuring that individuals with intellectual disabilities receive residential-based health, safety, and habilitative services in compliance with Intermediate Care Facilities (ICF) guidelines, the Omnibus Budget Reconciliation Act (OBRA), state and federal regulations, and facility policies. This position requires a working knowledge of habilitation practices, the ability to obtain First Aid certification, and completion of in-service training on individual problem-oriented treatment plans during the probationary period. Responsibilities include supervising assigned staff (e.g., Habilitation Providers) participating in the development and modification of Individual Habilitation Plans (IHPs). Typical staff size is 13-14 employees. Performs the material and substantial duties of the classification more than 50% of the time.

Summary of Job Duties

Supervises Habilitation Providers (HPs), ensuring treatment delivery and consistent implementation of IHPs, behavior programs, and integrating services with recreation, Occupational Therapy (OT), Physical Therapy (PT), speech therapy, and recreation services. Ensures residents active participate in programming groups, promotes their safety and well-being, and administers first aid when needed. Conducts visual checks on each resident and reports Unusual Incidents (UIs) and Major Unusual Incidents (MUIs), including pictures for UI statements. Assists with resident visits, ensures screening processes are followed and submits completed screening forms to Quality Assurance. Completes daily Habilitation Care Supervisor (HCS) checklist.

Provides instruction to HPs and agency personnel on specific programs and techniques, ensuring accurate and timely implementation of IHPs and required documentation. Facilitates Habilitation Services training during New Employee Orientation, oversees ten (10) days of floor training for new hires and completes detailed training checklists. Deliver ongoing in-service training to HPs.

Performs scheduling tasks in the absence of the Scheduler (weekdays after 3:30 PM, weekends, and holidays), coordinating with staffing agencies to maintain minimum staffing levels across all three (3) shifts. Monitors call-off logs, including nursing, and ensures coverage. . Oversees timekeeping through the UKG payroll system, approves leave forms and timecards and assigns staff for clinics, appointments, outings, and transportation duties.

Assigns, prioritizes, and monitors employee work performance and behavior. Conducts probationary and annual employee performance reviews, initiates follow-up for counseling, coaching, or disciplinary actions, and participates in interviews for prospective employees. Performs quarterly job audits and ensures clear communication with relevant personnel. Attends required meetings and performs tasks within the supervised functional area. Reviews documentation of residents' behavioral responses and progress per their IHPs. Monitors and updates CareTracker daily to ensure documentation is complete and checks shift reports to ensure employees are charting vital information for the oncoming shifts.

Assists HPs in delivering treatment and direct care to residents, as needed. Monitors the purchase and use of residents' personal belongings, including clothing, and ensures appropriate supplies and personal care items are available to meet both individual and group programming needs.

Monitors daily activity schedules, planned meals, and meal schedules. Protects residents' rights by promptly reporting and addressing any violations. Assists in maintaining accurate and current resident files and supporting ongoing family communication. Notifies supervisors, managers, or case managers of any specialized concerns involving residents, their families, or guardians. Submits maintenance, housekeeping, and safety-related work orders, through Facility Dude.

Minimum Qualifications and Requirements

High school diploma or equivalent, and four (4) years of related experience developing or implementing habilitation or treatment plans for persons with disabilities or severe to profound intellectual disabilities. This must include at least one (1) year of experience as team leader, or completion of three (3) college level courses in business or management; OR alternative, equivalent evidence of the minimum class requirements. Direct supervisory experience preferred (training, coaching, responsible for preparing evaluations, assigning work, responsible for the work of employees supervised).

Supplemental Information

Driving Requirement:

Must maintain a current, valid United States driver's license with an acceptable driving record.

Training and Development:

Competency based training in IDD/DD resident care; CPR and First Aid Certification must be successfully completed during new employee orientation. Must maintain certifications.
