As a case manager, you will make timely, independent decisions to support children and families with medical, educational, emotional, and social services. You will coordinate critical resources, ensure 24/7 emergency availability, lead family meetings, and guide families toward long-term permanency goals. This role involves regular in-home visits, close collaboration with caregivers, courts, and community partners, and a strong commitment to documentation and outcome tracking. Transportation, legal support, and attending court are also essential duties. Ultimately, you help ensure each child's safety, stability, and long-term success. This position is more than just a job-it's a commitment to walk beside children and families in their most vulnerable moments. Demonstrated experience as a positive contributor in a close team and community relationship, and with community resource mobilization.
