GENERAL SUMMARY: The Wellness Nurse LVN provides direct and indirect care to residents/patients in a sensitive, professional, and effective manner. The Wellness Nurse LVN performs his/her duties under the supervision of the SNF Resident Wellness Director while adhering to mandated nursing standards. The Wellness Nurse LVN is responsible for maintaining compliance with all state and federal regulations while utilizing the standards of care policies and procedures. ESSENTIAL JOB FUNCTIONS: Servant Leadership directs all aspects of decision making Contributes to all aspects of medication management, including but not limited to; orders, administration, storage, documentation and as mandated by the State Regulations and Guidelines as well as Company policy and procedure Assures resident/patient rooms and common areas are clean and safe at all times Assure that all resident/patient care specialists are familiar with the residents’/patients’ individual conditions and are trained to provide the resident services in an effective, professional and sensitive manner Continually assess residents/patients to identify any change in condition, mood, behavior, or level of function to ensure appropriate interventions to maintain the residents’ highest practicable physical, mental and psychosocial wellbeing Administration of medications Implements plans of care recommended by the Resident Wellness Director Implements change in condition plans Serves as the “Charge Nurse” for assigned shift ensuring residents/patients are well cared for Provides accurate and timely documentation of all care provided to residents Assembles and maintains resident charting and accurately maintains all records Completes accident and incident reports as required Maintains relationships with residents, family members and responsible parties Assist in the coordination of effective communication among the team,

residents/patients,

and families Be prepared for and manage emergency situations including disasters, fire and other emergencies Ensures adequate and appropriately trained team members are available for each shift within company and state guidelines Must be in company uniform and resident ready at all times Performs other related essential duties as assigned KNOWLEDGE, SKILLS AND ABILITIES: Able to communicate effectively with all levels of management, team members,

residents/patients,

family members, vendors, referral sources and outside contacts Able to make independent decisions Must be able to communicate in a warm, friendly and caring manner Must be familiar with and adhere to guidelines related to the Fair Housing Act (FHA) and the American with Disabilities Act (ADA) Must possess a passion to work with and around senior citizens Knowledge of customer service principles and practices EDUCATION REQUIREMENTS: LVN license Knowledge of the physiology and psychology of the older adult

EXPERIENCE PREFERENCES: Minimum One (1) year in the senior living environment PHYSICAL REQUIREMENTS: The following physical demands are representative of those that must be met to successfully perform the essential functions of this position: Able to stand or walk 75% of the day Able to concentrate with frequent interruptions Able to work under stress and in emergency situations Able to work under conditions that require sitting, standing, walking, reaching, pulling, pushing, and grasping Able to talk and hear effectively in order to convey instructions and information to residents and team members Occasionally lift/carry up to 50 pounds OCCUPATIONAL EXPOSURE / PERSONAL PROTECTIVE EQUIPMENT: Work in all areas of the community Subject to reactions from dust, disinfectants and cleaning chemicals and may be exposed to infectious waste, diseases and other conditions Use personal protective equipment and supplies when needed to prevent burns, falls, and infection Personal protective equipment includes infection control kit, rubber gloves, masks and non-slip shoes Subject to infectious diseases, substances and odors Follow Safety Policy & Procedures
