I. BASIC FUNCTION: To provide evidence-based psychotherapy and case management services to FCHC patients. All employees of FCHC must ensure service standards are delivered, including: FCHC Core • Demonstrates a commitment to FCHC mission and vision. • Demonstrates a positive attitude towards patients, employees, role, and the health center. • Demonstrates FCHC core values (accountability, courtesy, excellence, flexibility, integrity, respect). Customer Service and Professionalism • Smiles and makes appropriate contact, greets individuals upon entry into building and space. • Is customer service oriented to both internal (colleagues) and external (patients, clients, vendors, etc.) Customers. INTRADEPARTMENTAL RELATIONSHIPS: Department Director: Chief Behavioral Health Officer Reports to: Manager of BH Care Management Supervises: N/A JOB DESCRIPTION DEPARTMENT: BEHAVIORAL HEALTH SERVICES JOB TITLE: RECOVERY SPECIALIST ~ LCSW, LPC, OR LMFT RECOVERY SPECIALIST ~ LCSW, LPC, OR LMFT Page 2. III. PRIMARY RESPONSIBILITIES: 1. Conduct comprehensive behavioral health assessments to understand the unique needs and goals of patients. 2. Collaborate with patients to develop individualized treatment plans which include a life goal and a health goal, to build motivation and progress towards completion of these goals, and to track the completion of these goals. 3. Deliver evidence-based therapeutic interventions for a variety of behavioral and physical health conditions, including anxiety, depression, chronic pain and substance use. 4. Work as part of a team and communicate with primary care providers, psychiatrists, and other team members to integrate behavioral health services into overall patient care plans. 5. Coordinate information flow between the PCP, BH providers, CHW, and other FCHC staff within the Circle of Care program. 6. Maintain accurate and timely clinical documentation in compliance with FQHC and regulatory standards. 7. Attend BH team meetings, provider meetings, and site-specific meetings. 8. Provide crisis intervention and support for patients experiencing acute behavioral health issues. 9. Perform other duties as assigned. IV. PERIODIC DUTIES: 1. Contributes services to community public health activities as appropriate. 2. Participates in Center work team as needed. 3. Participates in Center staff problem solving groups. 4. Performs other duties as assigned. V. WORKING RELATIONSHIPS: Inside Center: All Inclusive. Outside Center: Patients, community organizations and agencies, professional colleagues and organizations, and institutions of learning. VI. QUALIFICATIONS: 1. Master's degree in Social Work, Professional Counseling or Marriage and Family Therapy required. 2. Eligible for professional licensure (LCSW, LPC or LMFT) within the first year of employment at FCHC required. 3. Previous work experience providing behavioral health services preferred. 4. Strong desire to work in urban community health setting preferred. 5. Strong interpersonal and communication skills. RECOVERY SPECIALIST ~ LCSW, LPC, OR LMFT Page 3. VII. CONTINUING EDUCATION: 1. Active professional development as demonstrated through attendance of seminars, continuing education programs, etc. is strongly recommended. 2.
