Position Summary: The Wellness Coordinator will lead community-centered outreach/resource tabling efforts focused on HIV prevention, harm reduction, and internal referrals housing navigation with an emphasis on supporting Life Is Work getting to Zero Initiative. This role combines direct service, outreach coordination, and data management, and is ideal for someone passionate about equitable access to health and housing resources. Additionally, this role is centered on providing HIV testing, HIV and PrEP/PEP/Doxy PEP linkage referrals to Life Is Work designated provider. Working as part of a dynamic, interdisciplinary team, the Wellness Coordinator will deliver life- saving resources (including but not limited to distribution of Narcan, condoms, lubricant and educational material etc. ), conduct community-based engagement via resource tabling, enroll participants in internal wellness programs, facilitation of Life Is work intakes, and support accurate data tracking through internal CMR system. Key Responsibilities: HIV Prevention Harm Reduction Lead HIV prevention outreach, education, and resource distribution in community settings. Provide Narcan (naloxone) distribution and brief overdose prevention training courses to community members and clients. Promote safer sex, safer use, and harm reduction practices with compassion and cultural humility.
HIV testing and linkage to care PrEP /PEP and Doxy PEP linkages and navigation Outreach Coordination Attend coordinated regular outreach events and community tabling to build awareness of organizational services. Foster and maintain strong relationships with community partners, grassroots organizations, and service providers. Represent the organization at public health fairs, community events, and coalition meetings as needed. Intake Services Assist clients with HIV testing, linkage to PrEP/PEP and Doxy PEP. Assist with linkage participants to STI testing and treatment Enroll eligible participants into our internal Life Is Work program using the CMR system under HIV testing and internal/external service referrals Connect participant to linkage providers for HIV, PrEP and Gender-Affirming Care Enrolling eligible participant in Medication Adherence Program to support viral suppression and/or increase adherence Coordinating providers appointment via in person/telehealth Data Program Documentation Complete accurate client intakes and service documentation in internal case management systems. Enter, track, and maintain housing-related data in CMR system Following HIPPA compliant protocols for data sharing purposes to support case conferencing Completion of referral forms for data tracking purposes Participate in all required training related to data privacy, HIPPA, mandated reporting, adherence training, PrEP training or any other mandatory training Participate in all required and suggested IDPH training.
Participate in all Case Worthy Training and other assigned/mandatory training courses Client-Centered Support Provide trauma-informed support with an understanding of LGBTQIA+ and gender- diverse populations. Connect clients with wraparound services such as legal aid, behavioral health providers, mental health service, nutritional support by way of solidarity closet etc. Attend and contribute to internal and external training opportunities, HIV trainings and prevention services training for harm reduction, PrEP, or vaccinations. Qualifications: Lived experience and/or Associate, bachelors degree in social work, public health, Community health, or related field. At least 12 years of experience in community outreach, HIV prevention, harm reduction, housing services, or DV survivor support. Certified HIV Risk-based Counselor/Tester Familiarity with harm reduction principles, overdose prevention strategies, and trauma- informed care. Experience using or willingness to be trained in internal client management systems (e.g., CMR). Strong interpersonal, organizational, and communication skills. Bilingual or multilingual skills are a plus Passionate about social justice, community health, and housing equity.
