Position Summary:
The Internal Admissions Coordinator is responsible for managing and coordinating the admission process for residents transitioning into the nursing facility from hospitals, assisted living settings, or other internal sources. This role ensures a smooth, efficient, and compassionate admission experience while maintaining compliance with federal, state, and facility-specific policies and regulations.
Key Responsibilities: Coordinate all aspects of the internal admissions process, including: Pre-admission assessments Bed availability Room assignments
Serve as the primary liaison between: Hospital discharge planners Assisted living partners Families and residents Clinical and administrative teams
Review medical records, physician documentation, and insurance coverage to: Verify eligibility Determine appropriate placement
Collaborate with nursing, therapy, and finance departments to ensure the facility can meet the resident's clinical and financial needs Schedule and conduct facility tours or admission interviews with prospective residents and families Facilitate completion of all required admission documentation and consents Maintain accurate and up-to-date records in the admissions tracking system (EHR/CRM) Provide ongoing communication with families during the transition to support resident satisfaction Monitor the initial adjustment period and escalate any concerns to the interdisciplinary team Assist the marketing or outreach team as needed with referral source relationships and census development Qualifications: High School diploma or equivalent
required Associate's or Bachelor's degree in Healthcare Administration, Social Work, or related field preferred Minimum
1-2 years of experience
in healthcare admissions, case management, or patient intake Experience in a nursing home, assisted living, or rehab facility strongly preferred Familiarity with
Medicare, Medicaid, and private insurance
guidelines Excellent communication, organization, and interpersonal skills Ability to work in a fast-paced environment, manage multiple priorities, and coordinate with clinical teams Compassionate, resident-centered approach and a professional demeanor Skills & Systems: EMR systems (e.g., PointClickCare, MatrixCare, or similar) CRM or admissions tracking software Strong MS Office skills (Excel, Outlook, Word) Knowledge of HIPAA and healthcare documentation standards
