Role Description

This is a full-time on-site role for a Social Worker based in Birmingham, AL. The Social Worker will be responsible for providing support to patients and their families through counseling, resource referral, and care coordination. Daily tasks include conducting assessments, developing treatment plans, collaborating with medical professionals, and advocating for patient needs. The Social Worker will also play a key role in facilitating access to community resources and services.

Qualifications

• Strong counseling and therapy skills
• Experience with patient assessments and developing treatment plans
• Ability to coordinate care and connect patients with community resources
• Excellent verbal and written communication skills
• Strong organizational and time-management skills
• Ability to work collaboratively with medical professionals
• Master's degree in Social Work or a related field
• Licensed Clinical Social Worker (LCSW) certification preferred
