Overview
BlueCross BlueShield of Tennessee is hiring a Social Worker. The role supports members\' physical, behavioral health, and social needs to help them live independently and achieve goals within their communities.
Key Responsibilities
- Partner with members and families to identify needed supports and direct services to meet personal goals for good health, employment, and independent or community living.
- Collaborate with a team of clinical and social support colleagues to address physical, behavioral health, and long-term service needs of each member.
- Conduct thorough face-to-face visits and assessments to determine current status and needs across physical, behavioral, functional, psycho-social, financial, employment, and independent living areas.
- Utilize criteria for authorizing appropriate home and community-based services and ensure services are being provided to meet members\' needs.
- Maintain valid driver\'s license and travel to members\' homes as required (mileage reimbursed).
- Available for an 8:00am – 5:00pm CST schedule; travel may be required for face-to-face meetings.
Must live within a reasonable commuting distance to Chattanooga, TN; travel to the Hamilton County area is expected.
You will be a good match if you have strong case management or behavioral health experience, solid communication and organizational skills, attention to detail, and the ability to create efficient, repeatable work habits. The role requires flexibility and willingness to travel for in-person member meetings.

- Conduct thorough and objective face-to-face visits and assessments to determine current status and needs across physical, behavioral, functional, psycho-social, financial, and employment areas.
- Authorize and ensure delivery of appropriate home and community-based services.
- Valid driver\'s license; immunizations or related medical tests may be required.
Job Qualifications
Education
- Masters in Social Work with an active unrestricted license (LCSW, LMSW, or LAPSW); or
- Registered nurse with an active license in Tennessee or in a state participating in the Nurse Licensure Compact; preference for those with CD Nurse certification (CDDN) for RNs but not required
Experience
- 2 years of relevant clinical experience required. Experience in behavioral health, child welfare, or intellectual and developmental disabilities preferred
Skills & Certifications
- Ability to work independently or in a team with shared responsibility
- Proficient in Microsoft Office (Outlook, Word, Excel, PowerPoint)
- Strong problem-solving and multi-tasking abilities with deadlines
- Excellent oral and written communication; strong interpersonal and organizational skills
- Organized, reliable, and able to manage multiple tasks with a strong work ethic
