Position : Unit-Based Clinical Pharmacist

Principle Duties: -medication order entry and verification, medication profile review for appropriateness, clinical services as needed by the specific patient population, and drug information. Participate in multi-disciplinary teams and/or rounds as assigned per practice area Perform drug therapy monitoring or other pharmacist-managed consults per protocol. For ED/critical care services, participate in acute patient scenarios such as cardiac arrest, stroke, sepsis, trauma, etc Location : State College, PA Schedule : variable days/afternoons (mainly day shifts). 8 hour shifts (mainly day shifts) IT : Meditech Comp range: Competitive. Relocation assistance available on a case-by-case basis. Ideal candidate : BS in Pharmacy or PharmD. PGYI OR 3 years of current clinical experience. PA pharmacist license or ability to obtain
