Location: Pensacola, Florida
Salary: $240,000 $270,000 + Performance-Based Incentives
Conduct H&Ps, periodic reassessments, and develop medical plans
Discuss care plans, diagnoses, and prognosis with participants and caregivers
Participate in intake evaluations and family/team conferences
Collaborate with interdisciplinary teams on chronic disease management
Provide leadership and strategic input as part of the senior care model
Order labs, diagnostics, and perform physical exams
Participate in on-call rotation (as required)
Educate participants, families, and team members on treatment options
Care delivered across multiple settings where participants reside
Active FL Physician License Required
Board Certified in Internal or Family Medicine Required
Board Cert. in Geriatrics Preferred
CDS & DEA Registration Required
Supervisory/Leadership Experience Required
1+ Year of Experience with Elderly or Frail Populations Required
Strong skills in physical assessment & chronic condition management
