Overview

Evergreen Commons Rehabilitation & Nursing Center is looking for a full time Social Worker for our long term care units.

Responsibilities

• Organizes and develops a plan of care based on the individualized needs of the residents. Assesses regularly to create goals and modify as necessary.

• Assists the residents, staff, and families with issues that impact or prevent adjustment or any other aspect of the residents' quality of life.

• Consults with other members of the interdisciplinary team to provide adequate services that enhance the social and emotional well-being of our residents.

• Consults with outside agencies if applicable. Responds to referrals and are initiated timely.

• Attends meetings as assigned.

• Responsible for providing services to families where problems may interfere with resident's plan of care.

• Completes documentation and assessments as required.

Qualifications

• Bachelor's Degree and at least one year experience in long term care.

• Organization and attention to detail.

• Must be able to communicate and interact with all members of the health care team.
