Overview
The VA Cheyenne Health Care System is hiring for one Social Worker for Inpatient Medicine/Hospice. Functional Statement #: PD565540
Responsibilities
- Provide direct clinical care, including case management and care coordination, for Veterans admitted to the Cheyenne VA Medical Center who are followed by the Hospice and Palliative Care team.
- Serve as an integral member of the Interdisciplinary Treatment Team (IDTT) and the Palliative Care Consult Team, delivering palliative and supportive services, education regarding decedent affairs and bereavement support, discharge planning, counseling pertaining to end of life needs, advanced care planning, symptom management, coping with loss of functioning, and coordinating inpatient and community hospice/palliative care and other needed services.
- Coordinate discharge planning from admission onward, especially for Veterans who will require nursing home, assisted living, or other community resources (including Non-Institutionalized Care), aligned with the Veteran’s and caregiver goals of care.
- Operate as an independent practitioner within the collaborative context of the IDTT, evaluating psychosocial needs and applying social work interventions with autonomy, professional judgment, and adherence to social work ethics.
- Provide counseling related to Veteran care needs, advanced care planning, coping with symptoms, and assisting families in adjusting to health care needs; coordinate post-discharge care with VA and/or community providers and other needed services.
- Act as a subject matter expert in health care decision making, including decision making capacity, Advance Care Planning, Advanced Directives, Durable Powers of Attorney for Health Care, State Authorized Portable Orders, Life-Sustaining Treatment Decisions, and Organ Donation.
Qualifications
- Must be licensed by a state at the independent practice level.
- Experience in medical social work; knowledge of community resources; ability to refer to community and other agencies and coordinate services; knowledge of medical and mental health diagnoses, disabilities, and treatment procedures to formulate a treatment plan.
- Knowledge of health care decision making and related processes; ability to review and align Advanced Directives with the Veteran’s care wishes; identify surrogate decision makers when needed and facilitate Goals of Care Conversations.
Work Schedule
Full-time, Monday-Friday 8:00am-4:30pm

- Paid Time Off: 37-50 days of annual paid time off per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year); credit toward annual leave accrual may be available based on prior work experience or military service.
