POSITION SUMMARY:
Facilitate the coordination of care by navigating patients through the medical and dental process to achieve desired outcomes
POSITION QUALIFICATION REQUIREMENTS Education:
Minimum high school graduate or equivalent
Graduate of a certified Medical Assistant program preferred
Experience:
Minimum three (3) years' experience in a clinical setting required. Minimum one-year EMR and patient scheduling experience required. Skills and Abilities
which may be representative but not all inclusive of those commonly associated with this position:
Excellent organizational skills
Experience with an EMR; Intermediate proficiency level with Outlook, Word and Excel
Good telephone skills
Good customer service and teamwork skills
Ability to maintain a professional attitude and demeanor at all times
Good verbal and written communication skills
Transportation Requirement:
Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri. License(s)/Certification(s) Required:
CPR Certification FUNCTIONS OF THE JOB
Essential Functions
Monitor a caseload of managed care patients for compliance with appointments, prescriptions, immunizations, education and health/dental care utilization
Communicate noncompliance to members of the health care and dental team for appropriate intervention
Schedule patient appointments, make reminder appointment calls, contact patients that have missed scheduled appointments.
Answers telephone calls in a professional and timely manner.
Assist clinic staff by faxing, scanning and receiving incoming information and distributing mail for patients from managed care plans.
Monitor and request patient medication refills, specified laboratory results and immunizations to ensure compliance
Contact patients for missed refills (asthma medications, prenatal medications, etc.)
Communicate with patients to determine barriers to care (transportation, access, etc.)
Work effectively with the healthcare and dental team to alleviate barriers to care
Track ER, hospital, and UC patient visits
Educate patients on appropriate use of ER/UC
Prepare and distribute mailings to patients
Work independently within a team environment
Provide a report of interventions completed and care gaps addressed
Effectively work with other healthcare and dental team members to ensure coordination of care
Assist with patient scheduling throughout the organization.
Provides back up support and additional training for Health Center Support Clerk and Patient Support Representative positions as required
Perform other duties as assigned
Marginal Functions:
Attend meetings
Assist in preparation of monthly reports
Assists in resolving patient complaints
