Position Summary:
This funded position is a great opportunity for a skilled professional with experience in healthcare workforce development, or community health. As the Program and Relationship Manager, you will lead several impactful statewide initiatives. You will oversee the planning, implementation, and execution of grant-funded projects. The first is
GROW (Growing Your Own Allied Health Workforce) -a three-year pilot program for Health Net providers in Sacramento, San Joaquin, and Stanislaus counties. Its goal is to strengthen the delivery of quality, culturally aligned care. You will also manage the
Cal Community Connect Project , led by the California Department of Aging (CDA). This pilot embeds trained Community Health Workers (CHWs) as No Wrong Door (NWD) Navigators within the aging and disability network. Your responsibilities include building and managing provider and community partnerships, coordinating externship readiness, and supporting scholar success. You will work closely with Area Agencies on Aging (AAAs) and Aging and Disability Resource Centers (ADRCs) to co-design CHW roles, workflows, and billing strategies. This role is ideal for someone with a strong track record in managing complex workforce initiatives. You should have excellent relationship-building skills and the ability to turn program designs into measurable outcomes. The position requires a self-driven, detail-oriented professional who thrives in collaborative settings and is passionate about strengthening healthcare and aging services workforce pathways across California.
Essential Job Functions:
Planning, Support & Execution
Play a lead role in planning and execution of HealthNet externship and CDA CHW integration activities, ensuring alignment with project goals, timelines, and deliverables. Identify local workforce needs and build partnerships with healthcare, behavioral health, and aging services organizations to advance program implementation. Coordinate with Futuro Health internal teams to design materials, workflows, and resources that support effective delivery. Organize and facilitate partner meetings, training sessions, and community events. Travel to provider and partner sites across California as needed. Project Duties
Maintain and monitor project workplans, deliverables, and metrics across both initiatives. Manage accurate records, including scholar participation data, provider engagement, CHW integration milestones, and financial expenditures. Prepare and present reports and updates for funders, institutional leadership, and community stakeholders. Provide ongoing communication and feedback loops with Futuro Health leadership to inform decision-making. Stakeholder Engagement
Develop and sustain strong relationships with healthcare providers, AAAs/ADRCs, funders, and education partners. Collaborate with providers on externship readiness, preceptor training, and participant support. Facilitate collaboration among CDA partners to co-develop job descriptions, workflows, documentation standards, and billing strategies for CHWs/NWD Navigators. Represent Futuro Health in external meetings, sharing lessons learned and best practices. Program Management
Oversee externship partnerships, ensuring providers and scholars are supported through enrollment, training, and placement. Coordinate delivery of CDA CHW training and technical assistance, including supervisor coaching and peer learning. Partner with Scholar Support and Externship-to-Employment teams to align scholar success strategies. Evaluation & Reporting
Collect and analyze program data to measure outcomes, including externship completion, employment placement, CHW integration, and billing sustainability. Implement monitoring systems to ensure progress and compliance with funder requirements. Prepare evaluation reports highlighting outcomes, challenges, and continuous improvement opportunities. Collaborate with Futuro Health's data and evaluation teams to ensure accurate, timely reporting. Financial & Resource Management
Monitor project expenditures, ensuring alignment with grant budgets and institutional policies. Contribute to cost models and sustainability plans for both externship and CHW integration initiatives. Assist with preparation of financial documentation for funders. Continuous Improvement
Identify opportunities to strengthen program design and execution across both initiatives. Apply best practices in workforce development, training delivery, and healthcare/aging services integration. Support the evolution of scalable, sustainable models for externships and CHW workforce pathways. Qualifications
7+ years of progressive experience in healthcare, aging services, workforce development, community health, or program management. Proven success in leading externship programs, clinical training, or community health workforce integration initiatives with measurable outcomes. Strong understanding of healthcare provider operations, aging services (e.g., AAAs/ADRCs), and Medi-Cal/Medicare billing and service delivery. Demonstrated ability to manage complex, multi-stakeholder projects on tight timelines. Excellent communication, facilitation, and relationship-building skills across diverse stakeholders, including providers, funders, education partners, and community organizations. High proficiency with office and project management tools (e.g., Microsoft Office, SharePoint, Teams, CRMs, project trackers). Adaptable, detail-oriented, and solutions-focused, with a continuous improvement mindset and ability to thrive in a fast-paced, grant-funded environment. Willingness to travel across California to support on-site implementation. Education
Formal education in healthcare, public health, workforce development, or a related field is a plus but not required. A bachelor's or master's degree may strengthen a candidate's profile, but equivalent skills and lived or hands-on experience are valued most. Supervisory Responsibility:
This role initially does not have supervisory responsibility over staff but may in the future.
Work Environment/Travel:
This is a remote role in a given geographical location and the employee will need to reside in California, preferably Northern California. Local travel will be required in addition to some travel to attend organizational meetings and conferences.
Physical Demands:
Able to sit, stand, and type for long periods of time. Ability to work irregular hours including after hours and weekends although this will be infrequent. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
