Market Physician Executive - Houston, TX
Market Physician Executive (MPE) – Houston, TX; Monogram Health seeks mission-driven physician leaders to improve patient well-being, quality of life, and health outcomes through in-home multispecialty care. The MPE will lead the in-home care model in the assigned market, collaborating with local teams of APPs, RNs, SWs, and PharmDs, and leveraging Monogram Health’s Multi-Specialty Platform and AI-driven care strategies to deliver in-home specialty care. This role reports to the Region President and drives clinical strategies, policies, programs, processes, protocols, guidelines, and operations to improve patient health outcomes within the market.
The MPE oversees daily clinical and business operations through delivery of direct patient care, care management services, social work support, and pharmacy services. The MPE reviews and approves care plans and directs treatment plans, and collaborates with community physicians, facilities, and partners in peer-to-peer and direct patient care decision making.
Roles and Responsibilities
Clinical Guidelines Execution – 25%
Know, understand, and deliver Monogram Health’s proprietary evidence-based clinical pathways.
Ensure adherence to established clinical guidelines and the Monogram Model of Care.
Review and approve plans of care for APPs, RNs, SWs, and PharmDs.
Maintain appropriate and timely patient documentation within Salesforce and Athena; review, approve, and co-sign APP encounters.
Clinical and Operations Performance & Quality Improvement – 25%
Own accountability for reducing total cost of care and Medical Loss Ratio.
Influence clinical outcomes including interventions, utilization, pharmacological management, multi-specialty platform performance, and HEDIS/Gap Closure.
Lead daily high-risk and concurrent review rounds.
Direct supervision of frontline clinical and operations team members.
Oversee and delegate operational responsibilities to the Market Manager for daily operations, patient engagement, scheduling, administration, strategic initiatives, and P&L management.
Regularly assess and present market performance and outcomes to executives and senior leaders.
Patient Care and Treatment – 25%
Provide direct and indirect patient care (including diagnosis and treatment of disease).
Engage with patients on treatment plans, collaborate with community providers, and follow evidence-based care pathways.
Conduct peer-to-peer reviews with community facilities and health plan partners.
Order labs, referrals, and execute actions to drive patient outcomes and close care gaps.
Population Health Management (PHM) – 20%
Provide clinical guidance to drive PHM activities, identify and intervene on high-risk patients, reduce admissions/readmissions, and improve quality of visits.
Collaborate with Medical Economics, Finance, and other stakeholders to address utilization trends.
Present population health strategies in clinical and operational meetings.
Conduct patient reviews targeting high utilizers and high-risk cohorts.
Miscellaneous
Participate in Monogram On-Call activities; needs vary with at least 7 days on call per quarter.
Provide coverage for other MPEs during PTO or vacancy, as needed.
Position Requirements
Willing and able to obtain hospital privileges at required facilities.
Remote within the designated market with occasional in-home patient visits and domestic travel.
Demonstrated experience applying evidence-based clinical criteria.
Experience in renal care and geriatrics.
Strong management and communication skills.
Active, unrestricted medical license in each state within the market; willingness to license in multiple states.
Experience with high-need Medicare Advantage and managed Medicaid populations.
Experience with NCQA, HEDIS, Medicaid/Medicare quality improvement, medical utilization management, and risk adjustment.
MD or DO degree from an accredited medical school; BC/BE in an ACGME-approved specialty (e.g., Nephrology, Internal Medicine, Family Practice, Emergency Medicine, Critical Care, Cardiology, Endocrinology, Hepatology, Geriatrics).
