Make rounds, record visits, and work with physicians. Administer injections, perform testing, record vital signs. Document resident status changes and acute episodes. Collaborate with Activity Director, DON, and Nurse Aides. Prepare and administer medications; order refills and chart accordingly. Complete assessments and discharge documentation accurately. Supervise medication self-administration per guidelines. Assist with activities of daily living as needed. Other duties as assigned. Qualifications/Skills/Educational Requirements Licensed LPN or RN required. Ability to lift at least 51 lbs and assist residents physically. Fluent in English (reading, writing, speaking). Experience with residents with memory impairments is a plus, especially in memory care areas.
