We are seeking a nurse practitioner (NP) or physician assistant (PA) to work full-time (5 days per week) doing primary care rounding in a skilled nursing facility in the Syracuse, New York, area.

Key Responsibilities
- Perform primary care in a post-acute setting for short-term and long-term patients
- Manage chronic disease, diagnose illnesses and medical conditions, and implement care plans
- Order and interpret lab results
- Prescribe and monitor medications
- Consult with physicians and coordinate patient care with other healthcare practitioners
- Educate patients and caregivers on coping skills for chronic and age-related conditions
- Build relationships with patients, family members, and staff to support effective care delivery
- Promote healthy lifestyle changes
Requirements
- Minimum of 1 year of experience as a nurse practitioner in a primary care setting required
- Post-acute care experience preferred, but not required
- Active and unrestricted New York NP or PA license required
- Active and unrestricted DEA with prescriptive authority required
