Job Description

Performs difficult professional work involving the coordination and provision of direct services to older adults who have experienced abuse, neglect, or exploitation. This position will be responsible for the administration, development, implementation and management of the Enhanced Multidisciplinary Team (E-MDT) of Onslow County. Activities will include regular meetings, training, and team building to facilitate the development of cohesive team with a common mission of better identify and respond to cases of abuse, neglect, or exploitation as well as frequent public speaking to both large and small groups of community partners.

This is a grant funded position with an anticipated end date of 9/30/2026. The continuation of the position will be based on the end of the grant or future funding.

Position Responsibilities

  • Coordinate and facilitate E-MDT meeting no less frequently than monthly.
  • Assist with establishing and enforcing appropriate team policies and procedures.
  • Develop best practices, policies, training guides, and professional educational tools.
  • Complete/facilitate necessary trainings pertaining to the successful implementation of multidisciplinary teamwork.
  • Identify service gaps and help resolve system problems.
  • Coordinate with community partners and team members to ensure accessibility and responsiveness of services and supports for older adults.
  • Collaborate with community partners and team members to coordinate and provide comprehensive care to adults experiencing abuse, neglect, or exploitation.
  • Serve as a connection on case presentation skills and debrief the team after meetings to improve future presentations.
  • Serve as a connection between community partners and team members to ease the flow of communication to help move cases forward.
  • Update team members regarding new services, programs, and relevant legislation.
  • Provide a forum for learning more about the strategies, resources, and approaches used by other successful E-MDTs.
  • Seek and screen cases that meet Intake criteria.
  • Coach team members on case presentation skills and debrief team after meetings to improve future presentations.
  • Document participation and case planning action items accurately while managing confidentiality and legal discoverability requirements.
  • Provide focused outreach targeting aging adults, allied professionals working with older adults, families, and caregivers county-wide.
  • Assist in grant reporting requirements.

Qualifications

Considerable knowledge of social work principles, techniques, and practices and their application to individual casework group work and community problems. Knowledge of the psychosocial, socioeconomic, and behavioral problems and associated treatment. Knowledge of governmental and private organizations and community resources. Knowledge of laws, regulations and policies which govern the program. General knowledge of medical terminology, disease processes and their treatment are required in certain programs or settings. Skill in establishing rapport with a client and applying techniques of assessing psychosocial, behavioral, and psychological aspects of client's problem. Ability to express ideas clearly and concisely and to plan and execute work.
Education, Experience, and Special Requirements

Bachelor's degree in in social work, or related field and moderate experience (1-3 years) in social work or counseling, preferably with the client population. Master's degree preferred.

Valid Driver License required
Excellent public speaking

Salary will be determined based on qualifications, internal equity, and market considerations.

North Carolina Local Governmental Employees' Retirement System
Eligible employees are automatically enrolled in the retirement system and must contribute 6% of their gross salary each pay period. Once eligibility requirements are met, employees will be able to receive monthly payments for life based on their salary, age, and years of service credit.

NC401k
The County contributes 1% of employees' gross salary into the 401(k) account regardless of the employee's deferral amount. In addition, the County matches employee deferrals up to an additional 4%.

Vacation Leave
Regular full-time employees earn 13.5 days of vacation leave per year. With each 5-year increment of employment, vacation accrual increases.

Sick Leave
Regular full-time employees earn 12 days of sick leave per year.

Paid Holidays
County employees receive paid holidays annually. The Board of County Commissioners approves and may adjust the schedule as deemed necessary.

Teladoc Mental Health Services
The County provides Teladoc Mental Health Services to help employees and their families with personal, family and job-related problems such as depression, and substance abuse concerns.

County employees are also offered a variety of supplemental benefits such as Life Insurance, Flexible Spending Accounts, 457b Deferred Compensation Plan, and MetLife Supplemental Plans.
