Travel Nurse Practitioner role at Monogram Health. Monogram Health is looking for a skilled Nurse Practitioner eager for the opportunity to make a difference in patients’ lives. The Travel Nurse Practitioner helps provide access to comprehensive care for underserved patients in the Monogram Health network, aiming to improve quality of life in the home and slow the progression of kidney disease and other chronic illnesses, enabling positive health outcomes.
Responsibilities
- Deliver personalized and compassionate care to patients primarily with CKD through ESRD.
- Serve as a nationwide float provider in areas where local clinicians are unavailable or where growth is rapid.
- Conduct patient health assessments and develop strategies to improve or manage health, promoting healthy habits.
- Maintain accurate patient records, schedule, and administer follow-up appointments across multiple markets.
- Coordinate care with primary care providers, specialists, and relevant ancillary services.
- Complete all documentation and paperwork in a timely manner and uphold quality of care standards.
- Work outside of a fixed office setting; must be based in the state of employment with periodic training/meetings outside that state.
- Deliver evidence-based, timely care to reduce avoidable hospitalizations, maximize quality of life, and prioritize patient health and satisfaction.
- Prescribe medications, order tests, and collaborate with the patient’s Monogram physician.
- Monitor patient outcomes and report improvements in quality outcomes daily.
- Help close gaps in care by educating patients about preventive monitoring and coordinating diagnostic testing with physician practices.
- Assist patients with enrolling in access to educational videos and participate in integrated care team meetings.
- Maintain knowledge of disease diagnosis and prevention; assess health status and implement appropriate care plans; conduct follow-ups and evaluations.
- Perform other duties as assigned.
Position Requirements
- Active and unrestricted Registered Nurse and Nurse Practitioner licensure.
- Board certification for appropriate licensure (NP: ANCC/AANP).
- Current and unrestricted DEA certificate, or eligibility.
- Ability to travel extensively by air and use rental vehicles.
- Ability to work autonomously without direct supervision.
- Access to transportation, a valid driver’s license, and car insurance.
- Proficiency with medical instruments and equipment required by the work.
- Knowledge of computer-based data management programs and information systems, as well as medical records and related technology.
- Effective verbal and written communication with retail and medical partners, patients, family members, physicians, and community representatives.
- Understanding of federal and state regulations including HIPAA and OSHA.
- Two or more years of direct patient care experience.
