Case Manager Location: 1100 S. Hamilton, Illinois Salary: $44,676–$47,650 (Based on Experience) Schedule :

We are TASC. Position Overview:

This position will provide addiction intervention, AODA counseling, and recovery support services to clients and their families. The Case Manager is responsible for bringing services, agencies, resources, and support networks together to assist clients in achieving their established goals. Key Responsibilities: Provide direct services and coordinate resources and people to help clients achieve their service plan goals

Screen and assess clients for appropriateness for agency services

Adhere to the agency’s service hour protocol

Support client engagement in drug treatment, community support, and other services

Counsel and aid family members to better understand and support the client

Facilitate individual and/or group orientation or educational sessions as required

Facilitate client drug testing when needed

Represent clients and the agency at staffings, court hearings, probation/parole meetings, and provider networks

Provide referrals for client needs and offer crisis intervention as necessary

Document assessments, service plans, progress notes, discharge summaries, and other client-related activities in a timely and accurate manner

Attend in-service trainings, conferences, and continuing education (as required)

Complete and maintain all documentation including ECR entries, letters, and status reports

Our Ideal candidate: High school diploma or GED equivalent Valid driver's license CADC certification within two years of employment with TASC may be required based on contractual obligations Demonstrated proficiency in the Microsoft Office suite Demonstrated experience in group facilitation Knowledge of the current ASAM placement criteria, DSM-V as it relates to substance-related disorders, and Code of Ethics for substance abuse professionals, or the ability to acquire the knowledge Flexibility in thinking and approaches to leading group discussions Good verbal, written, and interpersonal communication skills Good time management skills with a proven ability to meet deadlines Our benefits package includes: Medical/Dental/Vision/Life
