Job Description
Position Summary
The Social Worker provides psychosocial support and case management services to participants of the Adult Day Health Care (ADHC)/Community-Based Adult Services (CBAS) program. Working as part of the interdisciplinary team (IDT), the Social Worker assists participants and their families in coping with medical, emotional, and social challenges. The Social Worker develops individualized care plans, coordinates services, advocates for participant rights, and connects participants with appropriate community resources, ensuring compliance with Title 22, Medi-Cal, and CBAS program requirements.
Essential Duties & Responsibilities
Assessment & Evaluation
Conduct comprehensive psychosocial assessments of participants upon admission and at required intervals.
Identify psychosocial, behavioral, and emotional needs, as well as family dynamics and caregiver support systems.
Evaluate social determinants of health that may impact participant care and safety.
Care Planning & Intervention
Develop and implement individualized care plans in collaboration with the interdisciplinary team (IDT).
Provide counseling, crisis intervention, and supportive therapy to participants and families.
Assist participants in developing coping strategies to manage stress, illness, and disability.
Provide education and support to caregivers regarding participant needs and available resources.
Case Management & Advocacy
Facilitate referrals to community agencies, health care providers, and support services (e.g., IHSS, behavioral health, housing assistance).
Serve as an advocate for participants’ rights and access to necessary services.
Assist with transition planning when participants’ needs change, including hospital discharges, long-term care, or home support coordination.
Interdisciplinary Collaboration
Actively participate in IDT meetings, contributing psychosocial expertise to the development of participant care plans.
Coordinate with nursing, therapy, activity, and dietary staff to ensure comprehensive, person-centered services.
Provide in-service training to staff regarding psychosocial issues, cultural sensitivity, and participant support.
Documentation & Compliance
Maintain accurate, timely, and complete documentation in accordance with Title 22, Medi-Cal/Medicare, and CBAS requirements.
Prepare required reports, assessments, and progress notes to support regulatory compliance and participant care.
Ensure confidentiality and compliance with HIPAA regulations.
Qualifications
Education:
Master’s Degree in Social Work (MSW), Psychology, Gerontology, or Counseling from an accredited institution.
Licensure/Certification:
Licensed or license-eligible in California preferred (e.g., LCSW, LMFT, LPCC, or equivalent).
Current CPR/First Aid certification preferred.
Experience:
Minimum of one (1) year of experience working with older adults, individuals with chronic illnesses, mental health conditions, or developmental disabilities.
Prior experience in a health care, community-based, or adult day health care setting preferred.
Skills:
Strong interviewing, counseling, and crisis intervention skills.
Knowledge of community resources, entitlement programs, and case management practices.
Excellent interpersonal, organizational, and communication skills.
Ability to work effectively as part of an interdisciplinary team.
Working Conditions
Work performed in a community-based adult day health care environment.
Schedule is Monday–Friday, 8:00 a.m.–4:30 p.m., with flexibility depending on program needs.
May require occasional home visits or community outreach.
