SOCIAL WORKER Job at Prairie Winds Hospice in Shawnee

Master's degree from a social work curriculum accredited by the Council on Social Work Education.

Must have at least two years of experience working with the geriatric population. Two years of hospice experience is preferred. Strong communication skills and the ability to relate positively to others are essential. Must be able to work collaboratively with employees and volunteers, and effectively communicate the hospice concept to families, patients, community members, volunteers, staff, and other professionals.

II. Responsibilities

Work in the Kansas City area on the Kansas side. Must be licensed in the state of Kansas.

The Social Worker, as part of the Interdisciplinary Team, will:

• Assess the psychosocial and spiritual needs of patients and families.
• Evaluate patient insurance coverage and financial status; refer to appropriate agencies for assistance.
• Provide social services related to terminally ill patients and their families, including planning, treatment, and consultation with staff.
• Assist in developing and implementing the patient/family Plan of Care.
• Participate with Self-Managed Care Teams to support and educate patients and families based on hospice philosophy.
• Participate in IDT meetings, updating team members on psychosocial aspects of patient/family care.
• Provide counseling to patients, families, and hospice staff as needed.
• Relate positively to various personalities and family dynamics, managing reactions such as anger, hostility, or grief.
• Make appropriate referrals to community resources.
• Participate in bereavement follow-up services as required.
• Maintain accurate and timely records of all activities and services.
• Assist in training volunteers, staff, physicians, and community members.
• Help develop support groups and special programs as needed.
• Engage in CQI and Utilization Review programs.
• Work collaboratively as a team member at all times.
• Uphold the Hospice Code of Ethics.

III. Job Conditions

The position requires the ability to:

• Access patients’ homes and all rooms therein.
• Communicate effectively with patients and families verbally and in writing.

IV. Orientation and Training

Prairie Wind Hospice will provide orientation and training on hospice philosophy and procedures, which may be in-house or through outside seminars. Evidence of training will be documented in the personnel file.

V. Accountability

The Social Worker reports directly to the Executive Director.
