Registered Nurse Patient Navigator
The Nurse Patient Navigator for cancer services serves as a point of contact for referring physicians, patients, and caregivers guiding them through the complexities of the cancer care continuum within Froedtert South and the community. The Nurse Patient Navigator facilitates patient appointments, including those with lab, diagnostic areas, and specialty physicians and fields any concerns or questions families and/or patients may have to optimize the disposition of the patient and to act as the patient/family advocate. This role provides personalized support from initial diagnosis through survivorship or end-of-life care, ensuring timely access to services, reducing barriers, and improving the overall patient experience utilizing the nursing process which includes: an assessment of the patient's physical, psychosocial and spiritual needs.
Essential Duties and Responsibilities
- Guide patients and families through the cancer care continuum, from diagnosis to survivorship or end-of-life care.
- Act as a consistent point of contact, ensuring patients understand their care plan and available resources.
- Collaborate with the multidisciplinary team to coordinate diagnostic testing, treatment planning, and follow-up care.
- Provide education on cancer diagnosis, treatment options, side effect management, and supportive care services.
- Identify and address barriers to care such as financial concerns, transportation, language, or psychosocial needs.
- Maintain accurate and timely patient records within the electronic health record.
- Ensure compliance with hospital policies, accreditation standards, and oncology best practices.
Minimum Qualifications
- Current Registered Nurse (RN) license in the State of Wisconsin, or eligibility to obtain.
- Minimum of two (2) years of oncology nursing experience.
- Associate's degree in nursing required.

- Strong foundation in oncology nursing practice and a commitment to delivering compassionate, patient-centered care.
- Excellent verbal and written communication skills with the ability to collaborate across disciplines.
- Demonstrated ability to implement process improvements.
- Strong organizational, problem-solving, and decision-making abilities.
- Proficiency in electronic medical records (EMR) and related healthcare technology.
Working Conditions
- Work is performed in an outpatient environment with exposure to patients, families, and interdisciplinary staff.
- Moderate Noise (Ex.: typical hospital outpatient department, waiting area, or patient care area.
