Overview

Warren Haven, centrally located in Oxford (Warren County), is seeking a full-time Social Services Director to join our dynamic team. The Director will provide supportive and concrete services to residents and their families with compassion and intelligence.

Responsibilities

• Oversee the development, implementation, supervision, and ongoing evaluation of the Social Services Department to meet residents’ needs and help them attain or maintain the highest practicable well-being; ensure medically related social services are provided in accordance with State and Federal regulations.
• Coordinate preadmission services for inbound residents, participate in interdisciplinary evaluation of residents’ needs for institutional care, and complete the PASARR Level I.
• Facilitate residents’ safe transition back into the community through interdisciplinary discharge planning and arrangement of community-based services and follow-up care; assist residents and their representatives in locating and accessing financial, legal, and other community resources.
• Oversee procedures to ensure social services actions are documented in each resident’s medical record, with written recordings reflecting ethical and professional social work standards.
• Establish and maintain departmental QA procedures, and modify them as appropriate.
• Oversee Advance Care Planning for each resident upon admission, review any Advance Directives regularly, and ensure staff are aware of the resident’s code status and end-of-life wishes; educate residents and their representatives about healthcare options and implications.
• Assist residents in voicing and obtaining resolutions to grievances; review complaints, prepare written reports of actions taken, and ensure follow-through and resolution as per facility policy; track grievance trends and report findings as part of the facility’s QAPI program as requested.
