This is a combined posting for a Care Manager MSW and Care Manager LCSW. The requirements of each role are listed below under each associated title. The Issaquah School Based Care Manager is responsible for providing case coordination and a full range of clinical social work services including but not limited to psychosocial assessment, treatment planning, therapeutic interventions, crisis intervention and resource referral, within a high school setting. The School Based Counselor partners with the adolescent and when appropriate, his/her support system as well as connecting him/her with resources inside the school system and in the community. The social worker is guided by the NASW Code of Ethics and the Swedish Mission, Vision and Values in his or her daily practice.

Required Qualifications:
Master's Degree in Social Work from an accredited school
Upon hire: Washington Clinical Independent Social Worker Associate License
Excellent clinical experience and ability to manage clients in crisis
