Benefits:

Licensed Physical Therapist Position Highlights
Setting:
Home Health
Location:
Baltimore City & Baltimore County (assigned nearby areas as needed)
Units per Week:
30
Schedule:
Full-time, flexible hours based on caseload
Assignment Type:
Contract position with potential to hire based on attendance, performance, and business need
Remote/Onsite:
Onsite – Home visits
Licensed Physical Therapist Responsibilities
Perform physical therapy evaluations and treatments based on physician orders/referrals
Complete comprehensive assessments using appropriate evaluation tools
Review patient records for current diagnosis, medical history, and any precautions
Set measurable goals and develop a customized treatment plan to maximize patient independence and function
Reassess treatment effectiveness regularly and adjust plans as needed
Communicate patient progress and needs with the care team, physicians, patients, and families
Consult with medical providers regarding treatment plans and discharge
Recommend adaptive equipment and integrate its use into the rehabilitation program
Accurately complete and submit all documentation and billing in a timely manner per company, state, and federal regulations
Ensure compliance with HIPAA and corporate policies
Supervise Physical Therapist Assistants (PTAs) and Aides in accordance with state practice laws
Attend staff meetings and training sessions; complete required continuing education to maintain clinical skills
Licensed Physical Therapist Requirements
Licensed Physical Therapist in the state of Maryland
Experience in
home health care
required
Knowledge of
OASIS
documentation
Must be able to sit/stand for long periods and occasionally lift up to 50 lbs
Attendance is mandatory for the first 90 days
Licensed Physical Therapist Preferred Qualifications
Experience with
HCHB (Homecare Homebase)
and
Point of Care systems
Strong communication and time-management skills
Ability to work independently while maintaining high-quality care standards
