Overview

Family Service League is seeking a full-time Social Worker for the Regional Engagement and Addiction Program (REAP) in Mastic, NY. The Social Worker will provide support, intervention & case management to individuals, families and groups directly impacted by substance and/or alcohol use. The social worker will work within a multidisciplinary team to assist those struggling with substance use and other co-occurring disorders via street outreach and referrals from internal and external sources.

Schedule

Monday - Friday; Weekends and evenings as needed.

Responsibilities

  • The Social Worker will conduct community-based work that serves all of Suffolk County focusing on substance use disorders but not limited to the full range of DSM diagnoses, including mental health.
  • Work as part of a multidisciplinary team, partnering with case managers, Peers and CASACs to work collaboratively on clients needing assessments and linkage.
  • Screen for various determinants of health to provide appropriate support and referrals.
  • Make accurate diagnosis and confer with supervisor when needed.
  • Provide each client with realistic relapse prevention/crisis plan focused on harm reduction.
  • Coordinate and collaborate with FSL programs and community providers to link clients to community resources and other clinical providers.
  • Engage with outside providers to complete community/homeless outreach to provide harm reduction education and supplies, linkage to services, and support.
  • Provide outreach to clients in order to facilitate their engagement in services and offer additional support so they can engage in treatment and live successfully in the community.
  • Work closely with law enforcement to accept referrals and provide a co-response to individuals who are homeless or have substance/alcohol concerns in the community.
  • Acquire knowledge of modalities relevant to client population.
  • Conduct individual and group interventions with individuals and their families.
  • Provide de-escalation, clinical assessment, and linkage to individuals in crisis.
  • Provide telephonic as well as face to face outreach, engagement, and service planning
  • Provide opioid overdose education and train individuals and families to use Narcan.
  • Coordinate care planning with other providers of services/resources to ensure goal directed, collaborative care, including care transitions.
  • Act as a resource to all team members on psychosocial and/or substance abuse issues and resources.
  • Attend events in representation of the agency, including resource fairs, etc.
  • Maintain documents, records, statistics, and other related reports in an organized, timely and accurate manner as per policy and procedure.
  • Complete trainings and maintain proficiency with de-escalation and crisis intervention techniques.
  • Assess situations to determine importance, urgency and risks, and make clear decisions which are timely and in the best interests of the organization.
  • Set priorities, develop a work schedule, and monitor progress towards goals, and track details, data, information and activities.
  • All other duties as assigned.

Qualifications

  • Master's degree in social work or mental health counseling required.
  • LMSW, LCSW, LMHC, or limited permit required.
  • At least six months of experience with substance use disorders required. Experience with co-occurring disorders preferred.
  • Experience with conducting community outreach preferred.
  • Excellent interpersonal, organizational, problem solving and verbal and written communication skills required.
  • Ability to work as part of a multi-disciplinary team and the ability to work closely with referral sources required.
  • Valid and clean NYS Driver's License required.

Physical Requirements

This position requires sitting and standing for long periods of time.
