Hours: Per Departmental Needs - Full Time

We service the community by partnering with local employers to provide employees access to a network of primary care practices. We are modeled after Concierge Medicine and go above and beyond to provide the highest level of customer service and care to our patients. The member's experience at HealthWorks is extremely convenient and accessible, with our locations being onsite or near site to the employer, and membership based. Limited provider panel sizes allow HealthWorks providers and clinical support staff to provide relationship-based care with extended appointment times and same day/next day access.

This position is for a practice based in Atlantic City, NJ that provides care exclusively employees of the local casinos in partnership with Unite Here Health

Summary:

  • Performs the role of Social Worker to support patients in the Care Management Program by coordinating care of patient in the outpatient setting. Functions as a key member of the health care team to provide patient-centered, comprehensive social services to members across the continuum of care.

Responsibilities:

  • Collaborates with the health care team to assess and resolve social barriers for patients in the ambulatory care setting.

  • Develops and implements a patient-centered plan of care to empower patients with chronic illnesses to improve health and self-management within the community.

  • Assists the patient in overcoming barriers to health by using effective assessment, goal setting and motivational interviewing skills.

  • Collaborates with inpatient Case Managers to promote safe and effective transitions to the next level of care.

  • Develops effective relationships with patients, care teams, and community-based providers (home care, case managers, etc.) to promote continuity of care.

  • Assists families and patients in crisis intervention and long-term care planning.

  • Supports patients with accessing services such as, housing, security, legal, transportation, finances and public assistance.

  • Responsible for following documentation standards according to established workflows.

  • Participates in continuous improvement activities.

  • Enhances own professional development and maintains formal and informal continuing education for self-development

  • Performs duties in accordance with Penn Medicine and entity values, policies, and procedures

  • Other duties as assigned to support the unit, department, entity, and health system organization

Credentials:

  • NJ License at time of hire (Required)

  • Dual PA/NJ licenses within 6 months of hire (Required)

Education or Equivalent Experience:

  • Master's Degree Social Work (Required)

  • 1+ years' experience in a healthcare setting and/or experience with crisis intervention and supporting patients with psychosocial barriers to health. (Required)

  • Bi-lingual (Required)

Salary Range:

$53,497.60 - $86,528.00/Annually based on 40 hours per week
