The Case Manager RN provides acute care case management and discharge planning services for adult patients. The role focuses on coordinating patient care, managing transitions, ensuring regulatory compliance, and collaborating with the interdisciplinary team to facilitate safe and efficient discharges.
Key Duties include:
- Conducting comprehensive assessments of patients’ clinical, social, and resource needs.
- Developing and implementing discharge plans to support safe transitions of care.
- Coordinating with physicians, nursing, therapy, social services, and external providers.
- Managing prior authorizations, appeals, denials, and utilization review processes.
- Applying CMS, Joint Commission, DRG, HIPAA, and regulatory guidelines to ensure compliance.
- Documenting care coordination activities accurately and timely in the medical record.
- Participating in weekend rotations (1–2 weekend days/month, with a weekday off in exchange).Ax
