The Social Worker is responsible for providing case management, discharge planning, and counseling services to residents and families of the Nursing Home. In addition, the Social Worker maintains contact with community agencies and individuals who can contribute to a comprehensive plan of care when appropriate, and within the context of maintaining confidentiality.
Education, Training & Experience
- Preferrably LMSW, MSW or BSW
- Knowledge of MDS 3.0
- Knowledge of SigmaCare
- Bilingual with good communication skills in both English and Spanish
- 2 Years of social work experience with older adults.
Duties And Responsiblities
- Work cooperatively with colleagues and other staff
- Assessment of the social, emotional and physical needs of the residents and their families.
- Exhibits knowldge of MDS 3.0
- Facilitate, participate and organize (as necessary) interdisciplinary care plan meetings.
- Maintain contact with community organizations to secure resources for residents.
- Timely documentation in medical charts.
- Participate in in-service education programs.
- Facilitate the grievance procedure upon resident or family request.
- Assists CCP team in formulating an appropriate discharge plan for residents returning to the community.
- Assumes all other duties as assigned by the Nursing Home Administrator and The Director of Social Services.
- Must have patience, tact, and cherrful disposition and enthusiasm, as well as willingness to handle residents based on whatever maturity level they are currently fuctioning.
Physical Requirments
- Must be able to move intermittently throughout the day.
- Must be able to speak English (language) in an understandable manner.
- Must be able to cope with the mental and emotional stress of the postiton.
- Must function independently, have personal integrity, have flexibility and the ability to work effectively with other personnel in Social Services and other departments.
- Must be good in mental health and emotional stability.
