This Social Worker is responsible for implementation and maintenance of the HUD/VASH program under the direction of the VA CWM Healthcare System located at the Central Western Massachusetts VA Medical Center in Leeds, MA.

Responsibilities

  • Assess and screen Veterans in the HUD/VASH program.
  • Develop psychosocial treatment plans, actively involving the Veteran, family members, and/or significant others.
  • Develop and maintain a catalog of VA and community resources.
  • Perform regular outreach activities to area shelters and other community agencies serving homeless Veterans.
  • Advocate on behalf of the Veteran and work to break existing barriers to treatment.
  • Provide crisis intervention related to substance abuse, behavioral issues, loss of housing, and other crisis situations.
  • Provide education related to VA and community resources, entitlements, and Advance Directives/Living Wills.
  • Coordinate VA and non-VA services and programs, including inpatient and outpatient substance abuse treatment, mental health, medical services, and in-home services.
  • Maintain records, data collection, monitoring of patient and program indicators, performance measurements, and organization of program procedures.

Work Schedule

Full Time 40 Hours per week, Sunday 7:30am-4:00pm, Monday-Thursday 8:00am-4:30pm.

Requirements

  • You must be a U.S. Citizen to apply for this job.
  • Selective Service Registration is required for males born after 12/31/1959.
  • Must be proficient in written and spoken English.
  • You may be required to serve a probationary period.
  • Subject to background/security investigation.
  • Selected applicants will be required to complete an online onboarding process.
  • Must pass pre-employment physical examination.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

Basic Requirements

  • United States Citizenship: Non-citizens may be appointed when qualified citizens cannot be recruited.
  • Education: Master’s degree in social work from a CSWE-accredited program. Doctoral degree may not substitute for the master’s degree.
  • Licensure: Must be licensed or certified by a state to independently practice social work at the master’s level.

May qualify based on the Grandfathering Provision as described in the VA Qualification Standard for this occupation (for current VA employees in this occupation).

Grade Determinations

GS-9 to GS-11 depending on experience, education, and licensure. See VA Handbook guidelines for specific KSAs and requirements.
