Job Description The Safer Foundation is a social service provider to individuals with arrest and conviction records. Through a full range of services, including case management, educational instruction and advocacy, we focus on preparing individuals for the world of work by helping them find and keep productive and meaningful employment.
General Summary : Safer Foundation received funds from the Illinois Department of Human Services (IDHS) to hire a Reentry Housing Navigator who will play a crucial role in our Reentry Housing. The Reentry Housing Navigator will be a member of a multi-disciplinary team engaged in outreach, referral response, brief screening, referral and intervention, case management, and warm handoffs, concerning individuals released from the Illinois Department of Corrections who present as homeless or have housing instability. The Reentry Housing Navigator will complete an intake assessment with the client and create a service plan. The Reentry Housing Navigator will also receive referrals from individuals identified through the CRSC call center, as well as other Reentry Navigators, and work in collaboration to ensure that all service needs are met. This includes benefits assistance with (Medicaid, SNAP, SSI/SSDI), mental health, substance use, primary care, parole mandates, employment needs, and other factors.

Essential Duties and Responsibilities: The Reentry Housing Navigator will engage the individual to perform a housing screen to identify the severity of his/her housing needs. Complete a full-service plan with the client Referrals are made and updated monthly to reflect appointment dates and keep appointments. Each client’s needs and desired services will be reflected and documented in the client’s service plan. Conduct intake assessment to identify immediate and short-term needs. Utilize Salesforce and/or other database(s) to register clients. Advocate for participants in areas including but not limited to reentry, at-risk for violence, and/or at risk for homelessness. Responsible for case management of 50 reentry housing clients. Identify various supportive services clients will need (i.e., transportation, SSI/SSDI, Medicaid, SNAP) The Reentry Housing Navigator will work amongst other reentry navigators, job readiness team, and treatment counselors, to coordinate services, assess progress, and troubleshoot barriers. Assist with resume development and mock interview as needed.
Tracking, Document, and Reporting Complete all client documentation in a timely manner of service delivery. Ensure accurate data entry by completing mandatory self-file reviews weekly. Engage in individual supervision with the manager, identifying any immediate client needs or barriers to the reentry work/tasks given. Maintain compliance with HIPAA, 42 CFR Part 2, and other privacy and confidentiality laws and regulations for clients' protected information. Attending internal and external multi-disciplinary staffing. Participate in all internal and external meetings, bringing forth innovative ideas and/or feedback supporting continuous quality improvement and service delivery. In Salesforce, monitor referrals and follow up bi-weekly to confirm compliance for all internal and external referrals. All Safer standards are adhered to, including productivity standards.
Required Knowledge, Skills and Abilities: Ability to represent Safer Foundation in a professional manner while also being able to develop a rapport with clients from diverse backgrounds Hands-on experience and self-sufficiency with Microsoft productivity tools, including Outlook (for email), Office (Word, PowerPoint, Excel), and online collaboration tools (video conferencing, Zoom/Teams, messaging). Passion for social justice. Knowledgeable about resources in the community for formerly incarcerated individuals Knowledge about housing resources and eligibility requirements for various housing types Knowledgeable about IDOC offender search and Cook County Sheriff Inmate Locator Knowledgeable about the Application for Benefits Eligibility (ABE) website and Manage My Case function Data entry skills Telephone etiquette skills Skilled in social perceptiveness. Skilled in active listening and conveying information to others effectively Skilled in critical thinking, judgment and decision-making Skilled in de-escalation techniques and conflict resolution Lived experience is welcomed.
Education and Experience: Bachelor’s Degree and 3 years of experience providing case management services in a social service setting, one of which must be case management specific to housing referrals and placements. May be substituted with an associate’s degree and 5 years’ experience providing case management services in a social service setting, one of which must be case management specific to housing referrals and placements. Experience referring clients to a network of community-based providers and resources.
