Position - Social Worker

Location - Keene, NH

Shift - General

Job role - Full time/ Onsite

Compensation - Salary + all benefits + Bonuses

Primary Responsibilities

• Transitions & Admissions — Facilitate smooth admissions/discharges for children and families; conduct tours, home/hospital visits, and assist with paperwork (Medicaid, Social Security); maintain admissions calendar and complete social histories & annual assessments; coordinate care planning and post-discharge support; serve as the main point of contact for families; promote engagement and ensure access to community resources; maintain EMR records and schedule family meetings (including evenings/weekends); organize events, newsletters, orientations, and parent education sessions.

Interdisciplinary Collaboration

• Advocate for residents’ rights and emotional well-being
• Participate in care planning and document social progress notes
• Ensure compliance with ICF/IDD regulations
• Coordinate assessments and individualized programs
• Audit documentation and serve on the Ethics & Human Rights Committee

Qualifications

• MSW preferred; BSW with 5+ years’ experience considered
• Strong understanding of disabilities and family impact
• Knowledge of community resources and support systems
• Excellent communication, problem-solving, and multitasking skills
