Overview

Woodcrest Rehabilitation & Residential, a facility located in Flushing, NY, is seeking a full-time Social Worker (BSW or MSW).

Responsibilities

• Long Term Care / Subacute experience preferred.
• Attend and participate in interdisciplinary care plan meetings.
• Complete MDS while developing and maintaining individual care plans.
• Interview and assess residents’ needs during the admission interview process.
• Work with the team to coordinate discharges.
• Work with residents to maintain psychosocial well-being.
