_*PT Optometrist*_
Jessie Trice Community Health System is seeking a board certified PT Optometrist that will deliver care in accordance to the mission, vision, and values of the organization. This provider will render comprehensive primary eye care to the center's patients in accordance with the optometry degree, privileging, and State of Florida, utilizing evidence-based quality care to deliver care under established protocols. Comprehensive primary eye care will include but not be limited to diagnostic and rehabilitative procedures for treatment of refractive anomalies, complex visual anomalies, ocular diseases and manifestations of systemic diseases. The optometrist will work with the team for data collection, review of reports, and meeting HEDIS and other clinical performance measures to deliver high quality care and for maintaining Joint Commission, Patient Centered Medical Home designation, and Meaningful Use.
*Details of Education and Skills:*
Doctor of Optometry (O.D.) Degree form an accredited learning Institution.
Active and clear Florida License and no prior licensing infractions in other states.
Current board certification in optometry.
Knowledge of medications and ability to instruct patients in correct usage.
Ability to assess, examine, and document key elements for chief complaints and diagnoses.
A minimum of 2 years experience in a Medicare, Medicaid and other State & Federal healthcare programs.
Must have excellent organizational and people skills, including community/provider relationship skills.
Must be computer proficient and experienced with EHR (EPIC).

Current CPR
Bilingual (English/ Creole and or English / Spanish) is a plus

Pay: $115,000.00 - $120,000.00 per year
