Responsibilities

Job Summary

Responsible for managing an assigned caseload, working collaboratively with patients, caregivers, healthcare providers, and external partners to coordinate care and ensure smooth transitions across the healthcare continuum, aiming for positive patient outcomes, high-quality care, and cost efficiency.

Essential Functions

  • Collaborate with patients and caregivers early in inpatient or outpatient episodes to prepare for discharge, including supportive care, end-of-life decisions, community resources, goal setting, and long-term planning. Conduct assessments and execute safe post-acute interventions such as home visits, behavioral health coordination, guardianship, and other services. Assess readmission risks and barriers to outpatient care, and assist with advance directives.
  • Facilitate smooth transitions from acute care to appropriate levels of care by communicating clinical information and care plans among healthcare providers and with patients and families. Ensure understanding of financial obligations and discharge plans. Communicate with payers for service authorization.
  • Promote collaboration among healthcare team members, patients, families, and others to ensure integrated, appropriate, and beneficial care services from admission through discharge. Maintain caseloads and contribute to case management metrics.
  • Act as a patient advocate, facilitating communication among all parties and monitoring care quality using standards and evidence-based practices. Utilize knowledge of laws, regulations, insurance, and practice guidelines to advocate effectively.
  • Perform other duties as assigned.

Education

  • Master's Degree in Social Work – Required

Experience

  • Minimum of 1 year as a Licensed Social Worker or successful completion of a health-related Master's level field placement – Required
  • Preferred 2 years in case management
  • Preferred 3 years as a Licensed Social Worker
  • Experience managing behavioral health patients – Preferred

Licenses and Certifications

  • Case Management Certification – Preferred
  • Current licensure as an LMSW, LSW, or LCSW is required
