Wilmington Health Oncology is growing and adding an outpatient only Clinical Pharmacist to our team. The ideal candidate will be a PharmD and CPP with prior Oncology experience and a strong desire to teach, train and interact with patient daily. Hours are Monday-Friday 800a-500p but scheduling is flexible. Title : Clinical Pharmacist -Oncology

Duties/Responsibilities: Provides comprehensive medication management and education for patients for new regimens of Wilmington Health outpatient Hematology and Oncology. Education may include but is not limited to treatment logistics, adverse events and their management, storage and disposal of medications, dietary/OTC medication interactions, reasonable expectations of treatment, and monitoring requirements Obtains patient’s consent for treatment Works with providers for the collaborative management of medications and conditions related to patients plan of care Responds to drug information questions from providers and other clinical staff. Reviews lab values, patient status and comorbidities and makes recommendations on adjustments in regimens per physician protocols. This may include dose adjustments and/ or holding medications. Responds to/ triages patients experiencing reactions or side effects during treatment and makes recommendations based on physician protocols Ensures medications are ordered and available for patient’s treatment Interacts with CMO, Oncology Division Chief, Director and Manager in developing services to improve quality and cost metrics. Assists in the development and implementation of policies related to safe medication use Collaborates with other Wilmington Health pharmacist(s) on services and projects as needed Participates in local, state, and national professional organizations as deemed appropriate Reviews regimens for alignment with biosimilars for insurance purposes and makes recommendations for adjustments for profitability Meets with the CMO at least semi-annually to evaluate performance QUALIFICATIONS Must be a graduate of an accredited School of Pharmacy with advanced training (Pharm.D.)
