DaVita is looking for a Licensed Social Worker (Hybrid) to work with our dialysis patients. You will not be assigned to a specific location and you must be willing to obtain licenses in multiple states (DaVita will reimburse the cost).

Responsibilities

• Schedule – Monday to Friday, 9:00am – 5:00pm (some flexibility).
• Provide psychosocial care for patients, including interventions, mandatory assessments, screenings and education.
• Manage a caseload (typical range around 115–125 patients, spread between clinics; may vary).
• Build meaningful, long-term relationships with patients and their families in an outpatient setting.
• Collaborate with a team that supports and relies on each other in a positive environment.
• Contribute to performance-based rewards based on individual and team contributions.

About the Role

DaVita seeks an empathetic social worker who can work independently with strong support from the social work team. You will help patients understand their rights and responsibilities and guide them in managing the physical, mental, emotional, and financial demands of End Stage Renal Disease.

Requirements

• Willingness to obtain multi-state licenses (reimbursed).
• Meets all state regulations to practice in a dialysis setting.
• LSW licensure required for all social workers.
• Master’s degree in Social Work (MSW) with specialization in clinical practice.
• Knowledge of government and private insurance programs.
• Basic computer skills (MS Word, Excel, PowerPoint, Outlook) and functional proficiency with DaVita applications within 60 days.

Salary

Salary/ Wage Range: $27.00 – $49.50 per hour
