Job description: Nurse Practitioner Job Description Job Overview Community Physicians is a well-established, patient-centered healthcare group dedicated to providing high-quality, compassionate care throughout Northern Illinois. We are currently seeking a dedicated and experienced Family Medicine Nurse Practitioner (FNP) to join our team and help meet the growing needs of our community. Position Summary: The Family Medicine Nurse Practitioner will provide comprehensive primary care services in local nursing homes. This position offers a rewarding opportunity to serve patients while also managing care for elderly and long-term care populations. Responsibilities: Deliver primary care services including health assessments, diagnosis, treatment planning, and disease prevention Manage chronic and acute health conditions in collaboration with supervising physicians Provide continuity of care by rounding at local nursing homes on a scheduled basis Order and interpret diagnostic tests and lab work Educate patients and families on treatment plans and preventive care Document all patient interactions in the EHR Coordinate care with other healthcare professionals and facilities Qualifications: Current Nurse Practitioner license in the State of Illinois National certification as a Family Nurse Practitioner (FNP-C or FNP-BC) Minimum of 1-2 years of experience in family medicine or primary care preferred Experience or interest in geriatric care and nursing home rounding Strong communication, critical thinking, and organizational skills Ability to work both independently and as part of a multidisciplinary team Compensation & Benefits: Salary Range: $100,000-$140,000 annually (Collections Based Pay Structure) Medical, dental, and vision insurance Flexible spending account (HSA) Paid time off (PTO) Retirement plan Serving Northern Illinois Join us and make a difference in both clinic and nursing home settings.
