Job Summary
Direct Patient Care & Case Management Provide psychosocial assessments, individualized treatment planning, and discharge planning for adult inpatients.
Conduct individual patient and family meetings.
Deliver psychoeducation to patients and their families regarding mental health conditions, treatment, and recovery
Collaboration & Communication Coordinate with community partners, including San Francisco Department of Public Health (SFDPH), Conservatorship Office, and other agencies.
Maintain up-to-date knowledge of community resources, including referrals for food, shelter, long-term locked psychiatric facilities, residential care, outpatient programs, and clinics.
Group Facilitation & Education May lead or co-lead psychoeducation groups as needed.
Participate in the training and education of medical students, residents, and other post-graduate trainees.
Clinical Consultation Provide clinical input and support to the multidisciplinary treatment team.
Assist with referrals and ensure continuity of care across treatment settings.
Contribute to ongoing assessment and improvement of treatment services.
Additional Duties Provide individual psychotherapy, outreach, and case management based on programmatic and patient needs.
Complete timely and accurate clinical documentation in accordance with LPPH, TJC, Medicare, Medi-Cal, and private payer standards.
Attend staff meetings and perform other duties as assigned.
Remain current with clinical service standards, infection control protocols, and national patient safety regulations, including CDC handwashing guidelines.
The final salary and offer components are subject to additional approvals based on UC policy. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement. The salary range for this position is $41.79 - $72.17 (Hourly Rate).

Department Description
UCSF's Temporary Employment Program (TEP) recruits and hires temporary employees for immediate clerical and technical support services to UCSF Departments and various off-campus locations. UCSF departments deploy temporary employees to work on special projects, fill in for regular employees who are on vacation or leave, or to temporarily fill a vacant position during recruitment. Frequently temporary employees become successful candidates for career and limited appointment positions.
Required Qualifications
Masters degree in social work, or related field from an accredited university. Demonstrated ability to provide case management in medical setting, including biopsychosocial assessment, mental status examination, comprehensive services navigation, and to provide crisis counseling, use motivational interviewing, develop rapport and advocate for patient's needs. Excellent documentation and clinical skills. Discharge planning experience.
Preferred Qualifications
Licensed in the State of California (LCSW, LMFT) Experience with populations diverse in ethnicity, gender, and sexual orientation. Experience with severely mentally ill.
