Under the direction of the Access Center Supervisor, the Access Center Specialist identifies and provides direct services to homeless individuals and families at the 40 Prado Homeless Services Center. They act as a client resource hub by providing referrals to services available in the community. Services may include assessment, information and referral, distribution of needed basic supplies, on-site client supervision, and transportation to necessary appointments when needed. The specialist works with multiple service providers to enhance, augment, and coordinate service delivery to programs. They assist clients in obtaining all necessary documentation to enable them to move quickly into housing navigation or other programs and eventual permanent housing. The specialist collaborates with hospitals and emergency services personnel, such as law enforcement and paramedics. They supports CAPSLO's mission by providing care and assistance to clients, encouraging and facilitating self-sufficiency.

Qualifications: Experience : At least six months of experience in direct human or health services, particularly with economically disadvantaged, physically/mentally disabled, substance abuse/dual diagnosed individuals, the elderly, and/or at-risk youth. Relevant education, volunteer experience, or other related experience may be considered. Preferred:

Certifications: CDAC, CNA, EMT, Psych Tech, Med Tech, or other relevant certifications. HMIS experience is a plus. Skills : Strong organizational and communication skills, ability to work collaboratively, and a compassionate approach to client care. Driving License : Driving for business is essential for this position. Must have dependable, insured transportation, and a valid California Driver License (mileage to be reimbursed) and acceptable driving record. A DMV printout and proof of insurance will be required. Language : Bilingual in English and Spanish desired, but not required. As an Access Center Specialist you will:

Perform HMIS intake and CES assessments for all assigned households. Act as a resource hub by providing referrals to community services. Offer one-on-one assistance to clients in obtaining necessary documents (e.g., birth certificates, social security cards) and help them open savings accounts and manage budgets. Assist clients with housing searches and accessing community services such as drug and alcohol, and mental health services. Manage client sign-in, data collection, and maintain program attendance records. Ensure a safe environment by monitoring the site and enforcing policies. Work with clients to implement their Individual Plans within the Day Habilitation program, integrating them into all program aspects. Inform management and team members about behavioral and programmatic issues. Complete progress notes, daily logs, timesheets, and other documentation accurately and timely. Use positive behavioral management techniques and communicate in a person-centered, age-appropriate manner to build trust and self-esteem. Create activities that promote community involvement, self-help, employment, and independent living, adhering to individual plans and group schedules. Handle Homeless Services documentation, including client messages, daily logs, warnings, incident reports, and other clerical duties. Attend and actively participate in required training, stakeholder team meetings, and supervision meetings.
