*Position Title:* Outreach Coordinator - Wellness Team *Location:* North Hollywood, CA
*Employment Type:* Full-Time
*Position Summary:* We are seeking a dedicated *Outreach Coordinator* to join our Medical Office and support the Wellness Team in engaging patients. This role plays a vital part in promoting preventive care by coordinating outreach efforts for wellness checks, annual physicals, and specialist appointments. The ideal candidate will be comfortable with patient interaction and possess strong organizational and data entry skills.
*Key Responsibilities:*
* Conduct outbound calls to patients to schedule wellness visits and annual physicals
* Coordinate appointments with specialists as needed
* Support company initiatives aimed at improving patient engagement and preventive care
* Maintain accurate records and perform data entry in electronic medical records (EMR) systems
* Collaborate with internal teams to ensure smooth scheduling and follow-up
*Required Skills:*
* Patient service and customer service excellence
* Project coordination and time management
* Proficiency in Microsoft Outlook and EMR systems
* Strong communication and interpersonal skills
*Preferred Qualifications:*
* Experience with outbound patient outreach and scheduling
* Familiarity with EMR platforms
* Ability to manage multiple tasks in a fast-paced environment
*Pay and Benefits*
The pay range for this position is $21.00 - $21.00/hr.
