Job Title: PD Social Worker

Pay Range: $35.71 per hour

Job Overview

The Licensed Social Worker - LSW will provide treatment, social work services and complex discharge planning needs on the Child/Adolescent Psychiatric Inpatient Unit.

Qualifications

Required:

• Graduation from an accredited graduate school of social work with an MSW master's in social work degree.
• State licensure is required by the NJ State Board of Social Work Examiners.
• Licensed clinical social worker (LCSW) or licensed social worker (LSW).
• One- or two years experience in a hospital setting preferred.

Essential Functions

• Conduct biopsychosocial assessments for all patients, engage families and community support services to assess patients' psychological, social, environmental and financial status and meet identified needs
• Develop treatment plans with patients and the interdisciplinary team
• Participate in interdisciplinary treatment team planning rounds to assess discharge planning needs, identify problems and communicate ongoing interventions
• Educate and engage patients and families about treatment, discharge planning, patient needs, as well as availability and eligibility criteria for programs, services and benefits to meet those needs
• Coordinate and advocate for community-based services to ensure the highest level of needed supports and wraparound services
• Document in the Medical Record all assessments, reassessments and interventions to enhance multidisciplinary communications and to meet regulatory and reimbursement standards
• Attend appropriate Social Work conferences/workshops to maintain CEUs and license-specific requirements
