Responsibilities
The Population Health Nurse will work collaboratively within the multidisciplinary health care team to provide comprehensive care for our patients. They will focus on disease education and management, improving quality of patient care and patient outcomes, expanding access to care, and assisting with care coordination. They will also coordinate the care of patients, communicate effectively, and collaborate with physicians and other health care professionals.
Requirements
Preferred minimum of 1 year of current clinical experience Must possess the ability to effectively communicate and deal tactfully with other personnel, visitors, patients, physicians, customers, and the general public. Possess effective communication and interpersonal skills Have the ability to work independently Have the ability to function as a member of a multidisciplinary team, handling a variety of tasks and duties Must be flexible, have personal integrity, and the ability to function effectively as a team with other employees. Must demonstrate excellent problem-solving skills Must handle confidential material with sensitivity, maturity, and discretion Have competency with electronic medical record system and knowledge using clinical applications Must have current RN License Must have current Healthcare Provider CPR/BLS upon hire
