Assist residents with medications under the direct supervision and delegation of the Director of Patient/Health Services. Provides assistance to residents, as assigned, who need help with ADLs in a discreet and private manner.
DUTIES AND RESPONSIBILITIES
- Assist with the self-administration of medication as delegated by the Director of Patient/Health Services.
- Assist with self-administration of medication only if the Home Health Aide has completed and passed a Medication Assistance Training Program and is deemed competent by the delegating Director of Patient/Health Services.
- Check for expiration dates on medications.
- Reorder medications through the resident’s chosen pharmacy.
- Read communication Logbook and consult with supervisor regarding any changes in resident medication instructions.
- Check Medication Assistance Record (MAR) to ensure all necessary changes are reflected and inform the Director of Patient/Health Services or Designee.
- Keep medication carts neat and clean.
- Replenish supplies on medication cart as needed.
- Provide medication reminders and assistance.
- Perform MAR review at the beginning of each month and as necessary.
- Advise the Director of Patient/Health Services or Designee when updated prescriptions are needed.
- Establish re-order schedule for residents who do not use the house pharmacy.
- Inform the Director of Patient/Health Services or Designee of any resident refusals of medications at any time.
- Maintain accurate count of all narcotics at the start and end of each shift.
- Check the accuracy of refrigerated medication at the start of the 7:00am-3:00pm shift. Ensure refrigerator temperature is maintained at 36-40°F.
- Assist with activities of daily living as assigned or needed, including bathing, dressing, toileting, oral hygiene, and nail care.
- Engage residents in meaningful activities to enhance their quality of life.
- Follow directions as requested by the Director of Patient/Health Services or LPN Supervisor.
- Report and record all pertinent medical or social changes observed to the Director of Patient/Health Services or LPN Supervisor.
