Base pay range: $60.00/daily - $150.00/daily. Mileage reimbursement is available. This role covers preferred areas in Central Nassau and Western Suffolk, NY, with open to other towns as needed.

Gurwin Healthcare System provides a full continuum of care on a 36-acre campus in Commack, NY, including Gurwin Jewish Nursing & Rehabilitation Center, assisted living, and independent living communities.

Position type: Per Diem (as needed).

Responsibilities

• Planning, coordinating, provision and evaluation of health care to patients admitted to the CHHA Program.
• Recognizing patient pain and taking appropriate actions.
• Coordinating services to ensure patient needs are met and reviewing reports from involved disciplines.
• Re-evaluating needs and plans; coordinating information with all professional team members and agencies involved in care.
• Establishing realistic goals and methods for goal achievement with patient/family and caregivers via a coordinated care plan; outlining professional services and support modalities.
• Delivering direct nursing services and documentation; maintaining current patient records.
• Supervising workers providing health and personal care services during periodic home visits and assessing care delivery against the care plan.
• Notifying the patient’s physician of any change in condition.
• Upholding a high standard of care and contributing to a dynamic work environment.

Qualifications

• Current New York State Nursing License.
• Baccalaureate Degree in nursing preferred.
• Previous Home Care experience required.
• Comfortable with laptop/tablet use for home visits.
• Physical requirement: ability to travel to patients’ homes for visits.
• Ability to make clinical and managerial decisions within the scope of professional nursing practice and to perform procedures independently.
