Job Summary

The Care Connections Nurse Practitioners focus on screening and preventive primary care services delivered in the home, community, and nursing facility settings. Provides needed care in environments where patients feel most comfortable and receptive, including home, nursing facilities, and “pop-up” clinics.

Job Description

The Nurse Practitioner will work primarily in non-clinical settings, providing medical care to diverse patient populations, including adults, geriatrics, pediatrics, and women’s health. Responsibilities include performing comprehensive assessments, ordering tests, formulating treatment plans, consulting with specialists, and documenting care.

Job Duties

  • Provide medical care and care coordination for various patient groups.
  • Perform evaluations, including history and physical exams, and address care gaps and preventive assessments.
  • Manage both chronic and acute primary care issues, ascertain urgency, and establish diagnoses.
  • Seek specialist consultations when needed.
  • Order and interpret diagnostic tests judiciously.
  • Recognize scope limitations and refer to physicians when appropriate.
  • Create and implement medical care plans.
  • Schedule patient visits and coordinate post-discharge care.
  • Conduct face-to-face visits in various settings, including home and clinics, and via alternate modalities as needed.
  • Order targeted laboratory tests for specific populations.
  • Participate in community “Pop Up Clinics” to build community relationships and address health gaps.
  • Drive up to 120 miles daily within the region, with some overnight stays for special projects.
  • Obtain and maintain cross-state licensing as required.
  • Collaborate with team members to optimize work efficiency.
  • Prescribe medications and perform procedures as appropriate.
  • Document all care timely in electronic medical records.
  • May need to walk stairs while carrying equipment.

Qualifications

Required Education: Master's degree in family health from an accredited nursing program.

Required Skills/Experience: Proficiency in computer skills, including Word, Excel, and EMR systems.

Required Licenses/Certifications: Active national certification (AANP or ANCC), state license as a Family Nurse Practitioner, BLS certification, and a valid driver’s license.

Preferred Qualifications

  • 3-5 years experience as RN or Nurse Practitioner, especially in home or community health.
  • Experience managing chronic conditions in underserved populations.
  • Fluency in additional languages and skills in immunization/testing are pluses.
