Ambercare Hospice is seeking a compassionate and dedicated RN to join our newly restructured team in Santa Fe, NM. Responsibilities

  • Assume primary responsibility for patient and family caseload, including assessment, eligibility determination, admission processing, planning, implementation, and evaluation phases of the nursing process
  • Obtain data and assess information from patient physical, psychological, social, and spiritual factors that may impact needs and initiate intervention by other members of the interdisciplinary team
  • Initiate communication with the attending physician, interdisciplinary team, and other agencies to coordinate optimal care
  • Provide regular visits to reassess the patient’s needs and update the plan of care
  • Perform clinical pronouncement of death and provide support to the family
  • Maintain accurate patient records, charts, progress notes, reports, and prepare care plans timely
  • Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines

Qualifications

  • Graduate from an accredited registered nursing program
  • Licensed as a registered nurse in the state of practice
  • Current CPR and First Aid Certification
  • Knowledgeable about quality assessment and assurance procedures
  • Valid driver’s license and proof of insurance
