Responsibilities
- Administrative Functions — Assist the Director of Nursing Services in directing the day to day functions of the nursing activities in accordance with current rules, regulations, and guidelines that govern the long term care facility.
- Participate in developing, maintaining, and updating written policies and procedures that govern the day to day functions of the nursing service department.
- Ensure that reference material (i.e., PDR'S, regulations, professional standards of practice, etc.) maintained at the nurses' stations is current.
- Recommend written material that will assist the nursing service department in meeting the day to day needs of the resident.
- Ensure that the Nursing Service Procedures Manual is current and reflects the day to day nursing procedures performed in this facility.
- Ensure that all nursing service personnel comply with the procedures set forth in the Nursing Service Procedures Manual.
- Make written and oral reports/recommendations to the Director as necessary/required, concerning the operation of the nursing service department.
- Periodically review the department's policies, procedure manuals, job descriptions, etc. and make recommendations for revisions to the Director.
- Assist in developing methods for coordinating nursing services with other resident services to ensure the continuity of the residents' total regimen of care.
- Ensure that all nursing service personnel are in compliance with their respective job descriptions.
- Participate in the development, maintenance and implementation of the facility's quality assurance program for the nursing service department.
- Participate in facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing Services.
- Periodically review the resident's written discharge plan and participate in updating it as required. Assist the Director in planning the nursing services portion of the resident's discharge plan as necessary.
- Meet with the nursing staff, as well as support personnel, in planning the department's services, programs, and activities. Admit, transfer, and discharge residents as required. Complete accident/incident reports as necessary.
- Interpret the department's policies and procedures to personnel, residents, visitors, and government agencies as required.
- Perform administrative duties (e.g., completing medical forms, reports, evaluations, studies, charting, etc.) as necessary.
- Agree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations to the Administrator. Agree not to disclose resident's protected health information and promptly report suspected or known violations to the Administrator. Report any known or suspected unauthorized attempt to access the facility's information system.
- Committee Functions — Attend various committee meetings (e.g., Infection Control, Policy Advisory, Pharmaceutical, Quality Assessment) as required. Prepare written and/or oral reports of the nursing service programs and activities. Provide information to the Quality Assurance and Assessment Committee. Participate in developing the agenda for the Care Plan Committee meetings as necessary. Participate in reviews of the discharge plans and make plans available to the Care Plan Committee as required. Implement recommendations from established committees as instructed by the Director.
- Personnel Functions — Ensure a sufficient number of licensed practical and/or registered nurses and certified nursing assistants are available for each shift. Develop work assignments, delegate administrative authority to Charge Nurses, and ensure supervision and compliance with job descriptions. Ensure all nursing assistants are enrolled in or have graduated from an approved program. Supervise daily rounds, monitor absenteeism, and report problems. Ensure policy compliance, review complaints and grievances, and maintain good interdepartmental relationships. Create a warm, calm environment and participate in regular shift meetings to identify and correct problems. Participate in performance evaluations and staffing decisions. Administer disciplinary actions fairly and in compliance with policies. Report occupational exposures and suspected fraud as required. Ensure secure handling of resident information and compliance with privacy and security policies.
- Nursing Care Functions — Inform staff of admissions and room assignments, greet and escort new admissions, and coordinate rounds with physicians as needed. Plan and coordinate resident care with residents, families, and physicians. Monitor changes in resident condition, ensure direct nursing care by licensed personnel, review nurses’ notes for accuracy and alignment with resident wishes, and provide direct nursing care as necessary. Start IVs, collect specimens as ordered, and perform routine assessments. Manage admissions, transfers, and discharges, and report problems to the Director. Assist in corrective actions and keep the Director informed with timely reports. Monitor seriously ill residents in collaboration with the Charge Nurse and oversee medication administration, treatment schedules, and documentation. Ensure safe use of restraints per policy and that staff comply with medication procedures and infection control standards. Maintain clean, safe nursing areas and ensure residents can call for help as needed.
- Staff Development — Develop and deliver in-service trainings, assist in annual in-service planning, plan clinical supervision for nurse aide trainees, and participate in continuing education to maintain licensure.
- Safety and Sanitation — Develop safety standards, ensure training for exposure-related tasks, maintain PPE supplies, enforce hand hygiene, participate in drills, and contribute to infection control programs. Ensure safe operation of equipment, maintain MSDS records, and ensure safe handling of hazardous chemicals. Monitor compliance with safety regulations and maintain clean and sanitary work areas.
- Equipment and Supply Functions — Advise on equipment and supply needs, maintain adequate stock levels, assist in preventive maintenance, and ensure safe operation of equipment. Ensure proper use of supplies and that MSDSs are on file.
- Care Plan and Assessment Functions — Participate in assessments and care plans, encourage resident participation, review plans for goals and relevance, ensure staff refer to care plans, and assist with MDS planning and RAPs/Triggers. Ensure care provided aligns with the plan and wishes.
- Budget and Planning Functions — Stay informed of economic conditions, suggest nursing service adjustments, identify equipment needs, and report suspected fraud related to billing and costs.
- Supervisory Requirements — Assist with supervision and management of the nursing staff. As an RN, you are delegated administrative authority, responsibility, and accountability for assigned duties.
Qualifications
- Education and Experience — Nursing degree from an accredited college or university. Training in rehabilitative and restorative nursing practices. 2 years or more experience preferred in a long term care facility.
- Licenses and Certifications — Current, unencumbered active RN license in this state. Current CPR certification. Knowledge of PCC preferred. Maintain continuing education/licensing and stay in good standing with the State Board of Nursing.
- Language Skills — Ability to read, write, speak, and understand English. Able to read technical procedures and policy manuals and communicate effectively with managers and employees.
- Mathematical and Reasoning Skills — Ability to apply fractions, percentages, ratios, and proportions. Independent decision-making, problem-solving, and tactful interaction with residents, families, staff, and agencies. Demonstrated leadership and supervisory ability and ability to plan, organize, and implement programs.
- Other — Demonstrated knowledge of nursing and medical practices and relevant laws/regulations. Patience, tact, enthusiasm; willingness to adopt new methods. Able to relate information about a resident's condition. Must not pose a direct threat to health or safety.
- Physical Demands — Ability to move intermittently, communicate in English, cope with stress, see/hear as required, work independently, and meet health requirements. Able to lift/move at least 25 pounds and assist with evacuation as needed.
- Work Environment — Office and nursing areas, frequent interruptions, interactions with residents, family, staff, and government personnel. May require extended hours, weekend work, emergency call-backs, and exposure to various hazards. Willingness to participate in ongoing education and infection control matters.
